We begin with a conversation about needs, goals, routines, family priorities and preferred ways of communicating.
We consider the participant’s support needs, NDIS plan details where relevant, safety requirements and personal preferences.
We identify services that fit, such as daily living, community participation, transport, coordination, employment or assistive supports.
We confirm the support arrangement, schedule, responsibilities and communication process before services begin.
We stay connected with participants and support networks, checking whether the support remains helpful and appropriate.
